Outreach Caseworker Location: Newcastle Salary: £30,000 per annum Hours: 35 per week Contract: Fixed term contract (April 2026 - March 2027) with strong possibility of an extension for a further two years Additional Requirement: Applicants must have a full licence (2 years) and access to a vehicle. Do you want to help people navigate complex health and social care systems? Are you passionate about reducing unnecessary emergency service use and improving lives? As an Outreach Caseworker, you’ll work directly with individuals who frequently access urgent care services. You’ll help them identify unmet needs, build coping strategies, and connect with the right support. This role is vital in creating a more compassionate, efficient, and person-centred care environment. What will a day in the life of an Outreach Caseworker involve?
  • Supporting individuals with complex needs and/or smoking related respiratory complaints to reduce their use of A&E, 999, and mental health services.
  • Building strong relationships with health, social care, emergency services and local smoking cessation partners and community based respiratory health teams.
  • Creating personalised support plans and co-producing exit strategies with service users.
  • Attending multi-disciplinary meetings and advocating for service users across agencies.
  • Using your expert smoking cessation specialist knowledge and experience to support colleagues to have meaningful smoking cessation conversations with their clients and increase referrals and signposting into existing local smoking cessation services.
  • Identify gaps in community based services, relating to respiratory health, which may be contributing to, or exacerbating, frequent use of urgent care.
To be a successful Outreach Caseworker, what will you need?
  • Experience in casework across mental health, substance use, or other community-based services.
  • Experience of providing smoking cessation support and guidance.
  • Excellent knowledge of local smoking cessation services and referral pathways.
  • Strong communication and coaching skills to influence and support vulnerable individuals.
  • Confidence in managing safeguarding concerns and working independently.
  • Emotional intelligence, resilience, and a commitment to inclusive practice.
